This information is required for the purposes of assessing your eligibility for a tenancy. One form should be completed for every intending adult tenant.  Please answer the following questions as completely as possible.  Questions with a red star * must be answered.

FINALISING YOUR APPLICATION

BEFORE WE AGREE TO ENTER INTO A TENANCY AGREEMENT WITH YOU WE DO NEED TO UNDERTAKE SOME CHECKS.  PLEASE READ THE 'PRIVACY ACT STATEMENT' BELOW, AND INDICATE YOUR AGREEMENT TO IT.  UNLESS YOU DO THIS WE CANNOT UNDERTAKE THE CHECKS.

IF THE CHECKS WE UNDERTAKE ARE SATISFACTORY WE WILL ADVISE YOU AND PREPARE A TENANCY AGREEMENT FOR YOUR SIGNATURE.  PLEASE NOTE THAT YOU WILL NEED TO PAY THE FOLLOWING ON SIGNING THE TENANCY AGREEMENT.  KEYS WILL NOT BE ISSUED UNTIL THIS AMOUNT IS PAID IN FULL:

• BOND - TOTAL OF 4 WEEKS RENT
• A MINIMUM OF THE FIRST WEEK'S RENT

You will also need to bring along the following (if you haven't already provided them);
1. Personal ID  (Suitable ID: Photo driver’s licence, Passport, WINZ customer ID, Photo Credit card)
2. Proof of current address: (suitable proof: Power/Phone/Bank account)

PRIVACY ACT 1993 STATEMENT

If you do not supply all the information in this form, you may not be considered for this tenancy. You have a right to see the information held about you by any credit bureau or tenant default database and to correct that information in accordance with the Privacy Act.

This application form collects personal information about you, this information is:
(A) used to assess your eligibility as a tenant.
(B) If your application is unsuccessful this application form will be destroyed.
(C) if your application is successful this information will be:
   (i) retained in our office as part of your tenancy record.
   (ii) If you fail to comply with the terms of your tenancy agreement, and a judgement is entered against you by the Tenancy Tribunal pursuant to the Residential Tenancies Act 1986,this information may be provided to tenant default databases and/or credit bureaux. (A tenant default database records the names of the persons who have been found in breach of their tenancy agreement and/or the Residential Tenancies Act 1986 (RTA 1986) by the Tenancy Tribunal).
   (iii) This information will then be accessible to other people that may inquire of such credit bureau and/or tenant default database(s).

Acceptance
1. I authorise you to check my credit history prior to granting me a tenancy of the property.
2. I consent to you providing the information in this form and details of any proven breach of my tenancy agreement or of the Residential Tenancies Ac 1986 to any credit bureau or tenant default database.
3. I acknowledge that the information may then be made publicly available through credit bureaux or tenant default databases.
4. I agree that you may use any of the information on this application form to enforce any judgement in respect of the tenancy agreement or in respect of any order against me by the Tenancy Tribunal.
5. I confirm the information in this form is true and correct and that I have read the Privacy Act statement as stated above.

Sending this form to us will constitute your agreement that we can make enquiries of:
- the referees you have listed;
- credit bureaux;
- tenant default databases.

You will be asked to sign this form next time we meet in person.
